What Causes A Knot On The Head After An Injury?

My nephew hit his head on the ground after he crashed on his bike. (Kids bike) he hit it really hard. Has a knot on front of his head. But now he hasn t ate anything in 2 days bc when he does he gets really sick. Should I take him to the hospital. Is something seriously wrong?

The knot on the head is a bruise underneath the skin, but over the skull.

If he is acting differently, it would be helpful to have him evaluated by his doctor.

But the knot on the head would not be the direct cause of his sickness.
