What Causes Abdominal Pain With Distension Of Abdomen?

I am a gastric by pass pt. have no gall bladder. began with right side pain under ribs that radiates to shoulder. I then began having left side pain that is not intense that comes and goes. My abdomen is dissented. I have no fever or chills. I do I IBS and last night I was having lower abd pain from that. the Pain radiates to my back It comes and goes but the intensity is worse. I can not tolerate narcotics bc I have severe abdominal pain that feels like a gallbladder attck . Is this IBS or should I be concerned

Thanks for your query.
Noted the history of you being a patient of IBS and had gastric bypass, no gall bladder. Started with pain in the right side under ribs that comes and goes and also have right shoulder pain, similar on the left but with less intensity. There is bloating of abdomen and radiation of the pain to the back also.
Al such symptoms can come from pancreatitis, perforation of the intestine and so on.
I would advise you the following:
- Rush to ER as there may be an emergency.
- You need to be admitted.
Investigations of blood, urine, stool.
X-ray of the abdomen in standing position.
Ultrasonography and CT scan of the abdomen.
Intravenous fluids and antibiotics, supportive and symptomatic treatment.
If there is a surgical cause, you will need surgery, otherwise medical management will be sufficient.
