What Causes Abdominal Pain With Stomach Fullness?

My 25 yr old son is obese at about 460 lbs. Everything looked good as far as blood pressure and diabetes. Test results good. Scan of liver shows condensed and enlarged spleen. He is having issues of pain in abdomen and full ness and getting sick without notice. Waiting on new blood work results. What could be his problem? Dr. says liver disease as diagnosis so far. He never drinks or has smoked.
