Anal pain during sitting or coughing could be related to anal fissure or fistula or piles or infection around anal area but it can be commented only after digital examination of anal area and proctoscopy.
Since it is hpainful and if not getting better then you must need to get examined by surgeon to make clear diagnosis.
Without inspecting the lesion one can not say surely,so better you see surgeon who will inspect the anal region and prescribe you appropriate antibiotic like ofloxacin with ornidazole.
Meanwhile, you can take over the counter analgesics,anti-inflammatories and anti-allergic like antihistamines (cetrizine 10 mg) for symptomatic relief.
