Hello,
Welcome to HCM
from the information provided by you, it appears a rheumatic condition with possible underlying auto immune pathology. Consodering her age and medical histories of two bypass and associated conditions, it is a high risk case.
Incresed potassium levels need attention as it may predispose to cardiac arrhythmias. urine infection needs administration of higher antibiotics. Thyroid problem can be managed by external thyroid hormone supplements. Arthitic pain can be managed by anti inflammatory medication.
Considering all these facts, it is very much advisable to show the patient to a general physician and then to a specialist.

welcome for further suggestions.
