What Causes Bleeding While Urination?

in the past when using the bathroom, ive always had a little blood when pooping(on the toilet paper). but i was told that its ok and its usually tearing the skin alittle while coming out. well today i went and there was alot of blood it made the toilet water red, and it really scared me. when it came out it did hurt a little but thats just how it normally feels. is there something really wrong with me ?
