What Causes Blood And Gooey Substance In Urine?

I have had a lot of blood in the toilet after using the bathroom for about a month and thought that it was probably from a hemorrhoid but now I have blood plus this gooey substance that seems to be stretchy as well. It's very small in size and has happened a few times after using the bathroom. What could be the cause of this? Additional Information: I have had a lot of problems with my bowels and have started using stool softener because I go weeks without using the bathroom.
