What Causes Blood In Urine Along With Painful Urination?

I have pain in my back and when I urinate it fine into I get to the end then it hurts then a Little blood comes. It been that way for 3 days and I wake up out my sleep urinate and at the end a blood clot/ mucus like come out my urethral what could this mean

hai,
Blood in urine may indicate any urinary tract infection,stone.
at times poor hydration may lead to blood in urine most common in humid temperature especially in diabetic patients.

kindly do urine routine and urine culture to rule out the cause. blood clot may indicate some abnormality. so kindly do ultrasound too.
