What Causes Blood In Urine?

My 70 yr. old husband, had a complete prostatectomy 5 yrs. ago with a recurrence two yrs, ago, took radiation, psa 0..Starting passing blood 1 wk. ago, dr. did urine test, little infection, lots of dark blood, see urologist on Mon., taken meds. for 5 days, still passing blood today, woke up with blood on underwear????
