Pediatrician 's  Response
Hello,
Painful passage of hard stool associated with the passage of fresh blood could be due to an anal fissure due to constipation. Painless bleeding when passing stool means he is bleeding from the lower intestine. Causes such as Merkel's diverticulitis or intestinal polyp can be identified only after colonoscopy or specific investigations. Please consider this to be a medical emergency and consult a gastroenterologist immediately.

Dr. Sudharsana.
