What Causes Blue Discolouration Of The Nails, Fever And Increased Heart Rate?

My son has a stage four pressure sore which is healing slowly, six months now. He often gets blue nails, chills and slight fever. The only thing I can attribute it to is if the vac pump comes loose a little or he gets overheated. Seems like his body temp is not regulating very well since he was hospitalized six months ago for sepsis in the wound. Otherwise he is doing well and the wound is healing well with the vac pump. Just concerned for the blue nails, chills and fever and his heart rate is off the charts but BP is good. He is paraplegic so heart rate is usually 100 plus. Right now 148.
