As he is having this problem since long and now having fever requires some investigations like complete blood count with tC, DC, ESR, platelet count, routine urine check up and if require x-ray chest.
After all reports your doctor will have some conclusion about his problem and he will go for appropriate treatment.
Meanwhile give him paracetamol syrup for pain and fever.
Give enough water.
Ok and take care.
