What Causes Constipation, Abdominal Pain And Swollen Intestine In A Child?

My 5 yr old grandson has been battling chronic constipation since October 2016. He was hospitalized for 4 days in October. They said it was illeus. Since then he is still having chronic conspitation and tummy aches, loss of appetite, not gaining weight. Recent X-ray show still shows he has some bowel impact ion and swollen intestines. We have done numerous enemas, magnesium and miralax cleanses with little or no results. We feel like we are getting nowhere with his gi dr. Should we be concerned that he is not getting better?
