Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

* The absence of pus cells rule out possible severe infection underlying. As per my surgical experience, this can be from diabetes, gravels in the urinary tract, hydration level related issues or else.
* I recommend to get work up for diabetes and ultrasound of the abdomen & pelvis for final diagnosis and further management guidelines.

Hope I have answered your query. Let me know if I can assist you further.
