Internal Medicine Specialist 's  Response
Hi,

It seems you have diffuse parenchymal lung disease with your symptoms of dry cough, shortness of breath, chest tightness and history of exposure to fumes, gases (raw diesel fuel exhaust) and long history of smoking.

HRCT is superior to the plain chest x-ray (often appears normal on cursory look) for early detection and confirmation of suspected interstitial lung diseases (ILDs).

You should undergo diffusing capacity of the lung for carbon monoxide (DlCO). Glucocorticoid therapy is recommended for symptomatic ILD patients.

Hope I have answered your query. Let me know if I can assist you further.
