I had gone through the history given here, rectal lesion could be the cause of anal discharge although is very unlikely that it is happening after defecation only, the most possible condition could be anal fissure, inflammatory condition of anal region particularly rectum.
The only option you have is to see the surgeon and the clinical examination will rule out this.
