What Causes Enlarged Liver And Spleen Along With Diarrhea And Abdominal Pain?

17 year old male with air pockets in his chest, enlarged liver and spleen, sack of fluid around the appendix, diarrhea, sometimes bloody, abdominal pain, WBC normal, labs normal, no fever or vomiting, was complaining of lower back ache for 4 days prior to becoming ill. Ideas?

it is to be examined by surgeon stool examination should be done ca give norfloxacin tz twice daily for two days.further investigation is not requered. consult gastroenterologist for examination and advise on treatment.
