Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
Since, these are new symptoms and you have history of cardiac disease, I recommend you to visit a cardiologist and get an ECG, a cardiac sonography and some routine blood tests done. The swelling in lower legs and cough might be due to deterioration in cardiac functions. Your cardiologist will inform whether you need any additional tests, like a stress test to see if these 20% blockage is at the higher end or reduced.
All the best!
