General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

As far as my clinical experience is concerned, the condition is due to intestine motility related issue, intestine flora upset, narrowing of intestine lumen, neurology related conditions or else giving hard stools and fecal impaction.

Painful spasms are secondary to reflux pain via pudendal nerves as hard stool is not able to pass through external anal opening.

I recommend to immediately visit the ER unit and get treatment with consultant surgeon who will prescribe Dulcolax suppository or soap water enema after per rectal examination.

Do give me further chance to assist in future.

Thank you.
