What Causes Flank Pain On The Right Side And Blood In Urine?

My Grandfather is experiencing flank / abdominal pain on right side, with fever, blood in urine and confusion (seeing things that are not there) - he is 85 YO - initial tests say no kidney stones / issues and not appendicitis. Tests included blood work, CT scan, ultrasound.... Anything that jumps out as a distinct possibility here?
