you have not mentioned your age and sex,because so many things depend upon age and sex.
from your history it suggest that you may have some urinary infection,you may also have stone problem.so you are suppose to do urine test and if require should go for sonography.
for infection one course of antibiotic and alkaliser are require but for this you have to consult your doctor
