What Causes Intermittent Chest Pain And Tiredness?

In October this year I was experiencing occasional chest pain and just a dull tired feeling on my left side. So I scheduled a Dr s visit. An EKG and stress test was performed and I was told that I was fine (Oct 2015). I recently returned from several long direct flights overseas and my left leg is not feeling quite right. I am experiencing some dull pain and tiredness and also the same occasional chest discomfort. Breathing is fine. I am a smoker - trying to cut back and quit. I am also experiencing hot flashes (just finished my menstrual cycle). I have been previously diagnosed with NIS (Nerve impingement syndrome on the left) and had therapy for it twice. I have been to a spine specialist who informed me that I have some disc degeneration (C4, 5 & 6) I also seem to have anxiety. I have gained some weight but have not worked out in a few months because I was travelling and I also injured my left ankle. What should I do?

There are several reasons for chest pain, to simplify and summarize, about 40% of chest pains arise from gastrointestinal tract such as esophagitis, gastritis, peptic ulcer, etc. and about 30 % are the consequence and symptoms of cardiovascular diseases, about 25% derive from musculoskeletal disorders such as rib cage pain due to blunt injuries, muscle injuries, etc.

According to instruction you posted, it seems your pain is likely due to musculoskeletal disorders. However, we assume the chest pain originate from cardiovascular system until proven otherwise. In this perspective, to address your chief complaint and based my experience, you demand to consider on your additional signs and symptoms such as dyspnea, sweating wile feeling chest pain, whether chest pain accompanied with physical activities or not, having hemodynamic instability while facing to chest pain.

Since you have chest pain with a chronic Nerve Impingement Syndrome (NIS, which might be due to many reasons such as misalignment or subluxation of lumbar vertebrae, dorsal nerve injury and compression, being involved in a negative cycle of depression, stress and somatic nerve injury, etc), my recommendations are:

1.     Please do not ignore or underestimate your current symptoms, since may think your previous tests did not approve any cardiovascular disorder significantly and seriously.
2.     Repeat your cardiac reserve and stress tests using Holter monitoring(24h), stress echocardiography, However your cardiologist may order an invasive cardiac test such as coronary angiogram for advance search.
3.     You may be prescribed sublingual TNG to prevent chest pain potentially originated from cardiovascular system while feeling pain and chest discomfort.
4.     Routine urine, blood tests to analysis multiple organ functions.
5.     MRI, and CT Scan might be assigned after visiting your neurologist.
