What Causes Itchy Anus Followed By Secretion?

I am 46 year old male. 215 lb. 6 feet tall athletic. History of good health. I seem to have a chronic condition where the area around my anus gets irritated, itchy, and then starts to secrete.(gets wet around the area). Any thoughts as to what this is and how I can get rid of it. I saw a Doctor a year ago and he thought it was some kind of an immune defficiency.

Fecal soilage (for example, fecal matter leaking on underwear or other underclothing)
Type 1 and type 2 diabetes, Recent antibiotic use
Chronic dry skin conditions such as psoriasis and seborrhea, Profuse sweating and pinworm infection etc...
