General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history and understood your concerns.
It looks that you have holes near anus and not in the stool, also have itching and thus I would advise you the following in such a situation:
Consult a General Surgeon for clinical evaluation, examination, per-rectal digital examination, ano-proctoscopy and plan for colonoscopy.
If this is due to fistula in ano, you need a biopsy, fistulography and surgery along with biopsy of the specimen for getting the correct diagnosis for getting a specific treatment.
If there is no specific cause that can be found surgery still helps.
But there is no fistula but perianal excoriation then the obvious treatment is a course of an antibiotic, metronidazole, probiotics and local application of liquid povidone iodine after washing with copious amount of water and keeping the area dry; avoid tight clothes and hot, humid atmosphere.
