What Causes Muscle Spasms In The Joints, Hands, Back And Tongue?

I am a 54 yr old woman and since I was a child I can remember every once in a while waking up with spams in the calves of my legs and/or bottom of my feet that I have to walk out. The past few years these have gotten more frequent and much more severe. I now get them in my shins, thigh joints, hands, back and even my tongue! When I have them now it's several a day for a few weeks at a time and then I seem to have a period, maybe a month, when I have only a few. I have talked to a couple of doctors about it but they seem disconcerned and have had me try all sorts of vitamin and mineral supplements (calcium, magnesium, vitamin D, potassiium, fish oil etc...), and I also drink plenty of water. They are getting dangerous in that I will be cooking and have a hot pan in my hand and the hand spams and I can't hold the pot or I am driving and have to pull off the road because I can't control my foot, etc. I also suffer from RLS and take Requip for that (it helps), I was hoping that would help with the charley horses but it does not. Any help or suggestions?
