What Causes Pain And Bleeding During Bowel Movements With Itchy Anus?

My almost 3 year old son has a very red and itchy anis. He has had a little blood during a bowel movement and some reddish pimples dear the area. It started with just one a week ago and now there are maybe 15? He is afraid to do a bowel movement now saying ow and hasn t had one for 2 days. What could this be?

The possibilities here are -

1. Worm infestation
2. Habitual constipation
3. Anal skin tag for anal fissure.
