Hello,
1) As per your history, you might suffering from chronic constipation which require straining to pass hard stools
2) Excessive straining is responsible for development of piles and fissure
3) Pile is not associated with pain generally but fissure is always.
4) Pain in anal region, chronic constipation, hard stool - are associated with anal fissure
5) You need to consult general surgeon to assess severity of fissure; In acute stage, it require medical support and some local application of ointments to reduce pain and swelling; Whereas in chronic fissure just life-style and diet modification to prevent hard stool is sufficient
6) In rare cases, when pain is severe or fissure not heal with medical treatment then surgery is required
