What Causes Pain In Bum Leading To Difficulty In Sitting And Bowel Movements?

I have whats like a severe pressure in my bum. It sends shooting pains(that are bad) right up inside my bottom. It hurts to try to open my bowels or even sit down.
I also have a dull ache in my right arm. It hurts to lift anything or raise my arm. This has been going on for approx 3 weeks.
