An MRI is suggested to assess the internal structures of the knee joint.
As your knees are in bad shape I will advise for initial MRI and see the possible damage to the ligaments and soft tissues. If something can be worked out with the conservative management than a physiotherapist may be able to help you achieve good results.
In case the MRI reports and clinical assessment is not as expected than apart from conservative management the other options need to be looking out for.
Usually, the bump will be a fluid accumulation but cannot be a meniscus. Having a clinician examination and MRI will be good before taking any decision or coming to a diagnosis.
