Pathologist and Microbiologist 's  Response
Hello,

Since, you have a history of gout in past, pain in the first toe could be a recurrence of a gout attack.
I suggest you to consult your treating doctor for clinical assessment.
Change in medication may be required, since drugs controlling an acute attack of gout may be required.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Shailja Puri, Pathologist and Microbiologist
