What Causes Painful Heavy Period With Urinary Discomforts?

my 16 year old daughter started her period when she was 12 years old and her period has been getting more painful and heavier. It was normal for the first year. She cannot use tampons because they hurt and she is not sexually active. She also notices a lot of large clots of blood. Should we be concerned? She was a little shy to tell me, but she also said that when she has a bowel movement her tummy cramps when she is on her period and it is uncomfortable when she urinates. Thank You! T
