Hi,
How are you doing ? I understand your concern
The history you have provided is not sufficient to come to a diagnosis. I will need to know your cycle regularity, relation of pain to cycle, amount of bleeding, your sex life, amount of fluid seen- minimal or significant , so on. But let me give you a possible causes for the pain
Bulky ovary, with fluid collection in 21 yrs lady can be
1. Common & simple cause can be a ruptured corpus luteum hematoma/ cyst. around 14-16th day of cycle the follicle ruptures & releases the egg. The remaining part is called Corpus luteum. Sometimes blood can get collected in it(causing the ovary to be bulky) & cause pain. This collected blood may seep out of ovary & that fluid may be seen all around. But this needs only good pain killers & sedatives . wait & it will regress gradually regress. But if it isunbearable you will have to meet Gynecologist.
2. If sexually active - can be infection ( PID)around the uterus, ovaries. As an inflammatory reaction fluid collections can be seen in & around the uterus, ovaries & pain is very severe , especially around periods & during sex. Accompanied with white or yellow discharge, itching in & around vaginal opening, feverish feeling, thick darker blood during periods. This would need thorough examination by gynecologist , evaluation & management with good course of antibiotics for both partners
3. Bulky ovaries could be due to PCOD. Irregular periods, excessive body hair, hair fall, acne over face, darker shade over lower half of face, extra-pad of fat in mi-segment of body around thighs, breast, waist. Hump over the back of neck, dark skin over back of neck , inner part of thighs , below breast. Occasional periods with thick blood during periods or very minimal bleeding. This needs weight reduction with regular exercise & controlled diet, no fasting. Control cycle with hormones
4. If you had any hormonal stimulation for ovulation. They can exaggerate the follicle response & make ovary bulky, then you will have to wait for it to subside
5. Ovarian cyst- incidental. If very large >5cms, needs evaluation by gynecologist for management
Hope I have cleared your query, Let me know if I can help you more
All the best
