What Causes Persistent Swelling In Lower Legs And Feet?

I am 46 and have been obese most of my life. I am currently 342 lbs. My lipid panel is within normal limits, as is my blood pressure. I was diagnoses with MVP with mild regurgitation about 8 years ago after a trip to the ER due to an irregular heart beat that was not going back to normal. It eventually returned to normal after about 3 hours in the ER. I ve had persistent swelling of my lower legs and feet since my first pregnancy at 34 yrs old. I ve been on 12.5mgf daily of HCTZ ever since. Even so, I still swell quite a bit, especially on humid days. For the past few weeks, I ve been having pain in my chest, kind of like pinching pain in the area of my heart. It seems to happen at rest... most of the time at night while I m lying in bed. Could this be caused by the MVP?
