What Causes Recurrent UTI Despite Taking Antibiotics?

I have had several UTI,s in a short period of time. I have been in the hospital,I think four times (I have this documented ). I got very sick to the point of passing out . I went to the hospital by ambulance, I am taking a low dose antibiotic and I think I am doing ok, but I never had symptoms before, Can you tell me anything that might help?
