What Causes Repeated UTI With Chills And Fever?

I can not pass urine normally.i am on self cathetrisation and antibiotics.still i get repeated UTI with lots of chills and fever.my HB is going down,rbc is less, hematocrit is less than normal.my dr Ahlawat says that i will be fine ,but i find i am going down and down.Iam 75 yrs old male.i am afraid ,i may not develop kidney problem.Can anyone advice me,what to di?is there any treatment in US? Ifyes ,then what will be the cost?

Hello,

Causes of symptoms may be:
Absence of proper antiseptic precautions during self-catheterization, underlying urethral stricture, pH imbalance due to inadequate fluid intake, associated benign prostatic hyperplasia, faulty hygiene of genital. Systemic issues as diabetes/HIV/STD/others, kidney infections or stones in the urinary system, and others.

Very vital information lacking here is urination process at present, flow of urine, urgency, hesitancy, weak stream, night frequency, burning, and bleeding if any in urine. In absence of these I can guide as follows:
• Proper diagnosis of the cause is mainstay of the story then comes the management portion and cost.
• Need proper evaluation with expert urologist in terms of diagnostic cystoscopy, ultrasound of abdomen pelvis, laboratory work-up.
