What Causes Severe Abdominal Pain, Nausea And Fever?

My 55yo sister ate chicken and a bone stuck in her esophagus.Neck xrays showed bone in throat but when endoscopy was done the bone had migrated and could not be seen A small tear was however visible and left with no intervention. She stayed overnight and was discharged the next day eventhough she complained of severe abd pain, nausea and fever. Was sent home by RN without her doctor being informed about the pain. She returned to the ED 24 hours later and waited her obligatory three hours before being seen despite the fact that the staff were told about her recent procedure.By the time she was seen she was intubated, hypotensive and she died shortly thereafter. What could have been done for the tear during the scope.Does this kind of tear heal on its own? Thanks. Lilian.

General Surgeon 's  Response
Hello,

There are multiple possible conditions have to be explored herewith.
1. Chicken bone had passed esophagus and migrated down to stomach and small intestine, so likely to be responsible for further damage to the vital organs.
2. Slight tear seen during endocopy must be likely erosion from the chicken bone, which may not be required intervention as many of them heal on their own.
3. Procedure related complication does not usually show up after an overnight stay of normal observation without any symptom.

Most likely issue seems to be that of stomach or intestine perforation through the chicken bone which will cause all of the following
- Severe abdomen pain
- Nausea or vomiting
- Fever
- Signs of systemic involvement as rapid hypotension, breathless and need of an emergency intubation

However, your treating doc is best judge for the same and the final decision depends upon the autopsy report of the deceased.

Take care. Hope I have answered your question. Let me know if I can assist you further.
