What Causes Sudden Confusion And Fainting?

I have a very elderly client - recent exacerbated confusion, fall (no head strike), difficult behavior, recent tx (1/12 ago) for UTI, FWT was + leucs but MSU sent shows no growth but high polymorphs (532 and should be less than 10). What does this mean? CRP is normal, creatinine is raised at 156 umol/L as is Urea 16.7 mmol/L . Have encouraged adequate hydration
