What Causes Tightness In The Throat And Difficulty In Swallowing?

hi i am 39yrs of age...my throat feels tight all the time and its difficult for me to sleep.i also feel difficulty swollowing.at first i was told that all these could be symptoms of GERD.I was diagnosed as having a small hiatus hernia.i went for an operation and fixed the hernia through laprascopic surgery.it has not made any difference to my problem.sometimes my throat is very sore.it is even worse if i panic or think about it.i feel as if my throat muscle is always contracted.i cant drink anything that is cold.my throat gets even tighter.pls advise what shall i do.i do shift work and its hard for me to concentrate.sometimes i feel my jaws are sore as well.i also hear gurgling noise in my throat when i swallow my saliva.i take losec everyday.i know there is a problem...pls help
