What Causes Vaginal Bleeding After A Hysterectomy?

I had a hysterectomy and ovaries removed over 20 years ago so do not have periods. I also went through menopause at the same time. Today I went to the toilet, after urinating I wondered why it was continually dripping. When I checked the bowl was full of blood ans I was still dripping, when I wiped I had a couple of clots. It stopped after about 5 minutes but I must have lost at least a pint. What could cause this
