General & Family Physician 's  Response
Hello, it seems there is problem in in ur brother's pancreas. Pancreas is the organ which synthesizes insulin. So it can't produce insulin even when tablets are given. And thus insulin injection is necessary. But sometimes pancreas may not be completely destroyed and may produce some amount of insulin. Along with dat n may be due to irregular usage of injections der 'll be fluctuations in glucose levels n mood shifts. So I advice u to consult a diabetologist n have a proper advice. It would be better to take tabs to know if they 'll be effective or shift to insulin only. All the best.
