What Could Be Causing My Persistent Pelvic Pain, Cramping, And Bloating?

I have had months of pelvic pain, cramping, bloating. Pain is dull, and usually on lower left but occasionally elsewhere as well. CT scan without contrast was unremarkable. Positive cologuard which led to colonoscopy. Had 6 large polyps removed - 1 piecemeal and pathology shows the large ones were high grade dysplasia adenomas. Dr. wanted repeat colonoscopy a month later. No polyps found biopsies taken around site of prior piecemeal removal - awaiting results. Also had endoscopy which did not show anything significant.

Hello,

After removal of large polyps (especially piecemeal), you may experience persistent discomfort from the localized inflammation, scarring, or incomplete healing. Dull, crampy and left-sided pain will persist and is associated with bloating. After colon manipulation, some patients develop irritable bowel-type symptoms. IBS can cause cramping and bloating. You may notice it worsens with stress or certain foods.

Persistent or worsening pain despite polyp removal warrants ongoing study; it is good that a biopsy was done. Waiting for the biopsy report from the second colonoscopy is very important, as it can reveal hidden malignancy. Get a pelvic ultrasound or MRI done to rule out gynecological causes and get a pelvic f floor dysfunction evaluation done at the earliest. Try IBS therapy or low-FODMAP diet, it may be helpful.
