I went through your details. I suggest you not to worry much. You are posing yourself as having memory loss, Anxiety, headaches, sometimes short of breath, tiredness, no motivation and now suffering with clinical depression. Are these things diagnosed by a psychologist / psychiatrist?
If no please consult them. Self diagnosis may do harm to your self confidence and self esteem and of course, motivation.
I can help you in this aspect. Psychotherapy techniques should suit your requirement.
