What Do Painful Sores On The Left Foot Indicate?

I have 4 sores on the bottom of my left foot (2 the size of my Palm and 2 smaller ones the size of a half dollar) that are EXTREMELY painful, raw, swollen areas. This is causing me to not be able to put any pressure on my heel which is causing my ankle, calf, and knee to be painful. I also have a rash on my fingers that in some spots completely wrapped around the digit. My question is how concerned about this should I be and should I go to the er to be checked out
