What Does this pelvic Ultrasound Result Mean?

I recently had a pelvic scan done due to having pelvic pain, cramping, significant increased pain during menstrual period, back pain, diarrhea. Here are the results. Dr stated it only shows I may have been ovulating at the time of scan. I had my period 8 days after this scan and it has been over for 3 days. Still experiencing all of the symptoms described above.

Transabdominal and transvaginal images were obtained.

PELVIC ULTRASOUND, TRANS ABDOMINAL:

The uterus measures 8.0 x 3.7 x 5.1 cm. It is anteverted.

The endometrial stripe measures 8 mm transabdominally.

The right ovary measures 3.7 cm. It has a normal appearance without
evidence of mass lesion. Normal blood flow identified.

The left ovary measures 3.8 cm. It has a normal appearance without
evidence of mass lesion. Normal blood flow identified.

PELVIC ULTRASOUND, TRANS VAGINAL:

The endometrial stripe measures 12 mm transvaginally.

The right ovary measures 2.8 cm. It has a normal appearance without
evidence of mass lesion. Normal blood flow identified.

The left ovary measures 5.1 cm. It has a normal appearance without
evidence of mass lesion. Normal blood flow identified. There is a 3.2
cm follicle of the left ovary. A small amount of free fluid is
identified likely physiologic. It measures less than 3 cc.

General & Family Physician 's  Response
Wellcome to HCM,
There transvaginal ultrasound scan is more reliable as compared to transabdominal scan for pelvic organs.
The right ovary looks normal.
The left ovary shows a follicle with a diameter of 3.2 cm which is a positive finding in your case.
There presence of a cystic lesion like follicle in ovary over size of 2.5 cm is considered as cyst according to most of the literatures.
As in your case I will mention it as cyst rather than a follicle and will suggest to to undergo for another ultrasound scan to rule out if it was a dominant follicle or a cyst.
This will confirm if there is cyst or not and if it is there, its a positive finding and then you have to consult to a gynaecologist with the same.
