What Does this transvaginal Ultrasound Result Mean?

Hi Doc,
The results of my transvaginal shows below: do i have to worries?thanks.
Uterus is retroverted,measuring 5.5x4.4x4.5cm. There is a hypoechoic structure in the anterior aspect,measuring 0.7x0.6x0.7cm. The rest of the parenchyma is homogeneous. Contour is smooth. Endometrial stripe measures 1.0cm.

Cervix measures 3.5x2.5x2.8cm.

Right ovary measures 2.0x1.7x2.1cm with a volume of 3.9ml. Left ovary measures 2.4x1.5x2.6cm with a volume of 5.3ml.

There is a heterogeneous,rounded,well marginated structure in left adnexal area,between the left ovary and the lower aspect of the uterus,measuring 2.0x1.4x2.1cm.

Negative for posterior cul de sac fluid.

Impression:
Normal sized retroverted uterus with an intramural myoma
Thickened endometrial stripe,likely in secretory phase
Normal sized ovaries
Left adnexal mass, ovarian versus paraovarian
Negative posterior cul de sac fluid

1.There is a lesion in the uterus - which shows features of fibroid, does not need any active treatment - unless until any severe bleed like symptoms.
2.Generally treated with hormonal initially - depending on the age and reproductive status the operative treatment depends on.
3.There is a lesion in the pelvic region - heterogeneous in the nature - suggest towards the malignant features.
4.In this case the malignancy marker test is indicated to rule out the malignancy.
5.Cross sectional study is indicated to see the extensions and associated features.
