What Does Pain In The Knees Indicate?

my dad is 70 yr old, 350 lbs and has bad knees and pain. he is not a good candidate for surgery. His doctor use to prescribe a cream but factory that made either no longer makes or went out of business. I do not know name of med either. So I need to know a good cream he can use that will work for knee pain. His kidneys only work at 38% and he already takes a small dose of hydrocodone. Any suggestions for a cream or something that will work. He has bone on bone pain.
