Hello there,
Your dose of insulin depends on your weight if you are taking only insulin. However considering you are also taking medicines then the dose of insulin depends on your dose of medicines and your response to the medicines. However let me warn you that Lantus is not same as Huminsulin 50:50. They are two different types of insulin and should not be used as substitutes. The only way to tell the dose is if you check your sugars next day morning in fasting state. If sugars are under control then that is the right dose. Do discuss with your diabetologist about your doses after SMBG (Self monitoring of blood glucose) at different times of the day (like fasting, 2hour after breakfast, before lunch, 2 hour after lunch, before dinner and midnight) each test on a different day is enough to guide insulin titration. The way you are substituting the two types of insulin is not correct and may cause severe low sugars in the middle of the night.
Take care.
