Yes, sometimes it is difficult to pin-point the cause of such a pain which is left sided in the testicle, left lower quadrant and left kidney area which is enhanced before and after bowel movements; going on for 3 years and undiagnosed in spite of seeing many Specialists and including Cleveland Clinic.
The most probable causes I can think about are as follows:
Nerve entrapment syndrome at the spinal level which is enhanced by bowel movement as you may need to strain or there is nerve connection with the nerves of the bowel responsible for
peristalsis and thus interaction of the somatic with autonomous nervous system.
You need a help of a
Neurologist so that a Proper clinical evaluation, examination and MRI of the spine as well as of the retro-
peritoneum may help to get a diagnosis.
Possible treatment at the Pain relief center may also help in diagnosis as well as management.
Take a laxative to have smooth passage of bowel.
Try
Gabapentin or such medications although needs a valid prescription.
Stress and anxiety do add to such pains hence have a proper control.
