Diabetologist 's  Response
I am actually a little confused over your question.
1) Appendicitis Unless its chronic appendicitis is an acute emergency and surgery has to be carried out. Medical line of treatment is followed in appendicular abscess.
2) If sugars are not coming down with insulin please do the following
a) Check expiry date of insulin
b) Check if your uncle is taking insulin properly
c) Look for any lumps at the site of insulin injection
d) Make sure he is not eating sweets
e) Check blood sugars regularly and consult a diabetologist.
Am sure he will do well. All the very best!
