Known endocarditis with febrile fits needs a paediatric care facility to control the seizures with anticonvulsants and the antibiotics broad spectrum like ceftriaxone for coverage of organisms and a strong keen monitoring by a paediatric cardiologist for the infective endocarditis. Get the paediatrican and let them take care of the kid. It is manageable easily.
